Instructions:
Please arrive a few minutes early to allow time for building check-in. Upon entering, stop at the security desk on the left side of the main entrance to request an elevator pass. A valid photo ID is required for all visitors.

If someone will be accompanying you to your appointment, please let us know in advance. All visitors must be listed with building security ahead of time and must present ID to receive their own elevator pass - each person must have a pass to ride the elevator.

Once you have your pass, take the elevator to the 12th floor, check in at the front desk, and have a seat in the waiting room. Your provider will come greet you there.
